This archive contains protected health information. All searches and downloads are recorded and visible to all users on the Activity page.
Partial claim numbers work (e.g. WC2015). Claimant search is by surname. Case ID is the internal number (e.g. 30627).
Help
Finding a case. Search by claim number (partial is fine), claimant surname, or the internal case ID. Pick the case from the list.
Downloading. Each document has a Download link (valid for 15 minutes once clicked). Download all as ZIP packages every document for the case, with a report, and is ready in a few seconds to a minute.
Where the documents come from. Case documents (medical records, IME reports) came from the EVO file share; letters, invoices and email attachments from the EVO web server. Everything is stored immutably in AWS S3 with Object Lock until 31 December 2032. Filenames are the originals; they do not contain the case number, which is why this index exists.
Missing items. A small number of records (308 of 1,063,167) had no file anywhere by the time of archiving. They are shown greyed-out with "no file in archive". There is no other copy.
Emails. Email attachments are here; email bodies are only in the database export held by IT.
Privacy. This is PHI. Download only what a request needs, store it in an approved location, and delete it when done. Every action is logged with your name.
